The "T-34 Airworthiness
Questionnaire" (see this page for
details) has been superseded by the "T-34 Detailed Aircraft
History Questionnaire" (keep reading here).
Background
The information requested in this
Detailed Aircraft History Questionnaire (DAHQ) is essential in order for the holders
of the various AMOCS to complete the drafting of the terms of
those AMOCs and in order to return your aircraft to service.
The FAA has expressed serious
concern that there is a generalized lack of knowledge and
understanding about the nature and history of this fleet of T-34
aircraft.
In order to address that concern,
this Detailed Aircraft History data form has been generated. The
AMOC holders represented to the FAA that, as a portion of the
work being done to obtain approval of amended AMOCs to return
the aircraft to flying status, that Detailed Aircraft Histories
would be obtained in an organized form and reviewed for accuracy
as to the logbook and maintenance history by a mechanic.
You should make a diligent effort
to provide all of the information that is requested. If the
information is unavailable, please provide as much information
by way of explanation for the circumstances related to the
missing information as is possible.
You should have a mechanic with
inspection authority review your complete logbooks and other
maintenance records and then review the information provided
herein that is derived from the logbooks in order that it will
be as accurate as possible. You should not expect your IA to
“rubber stamp” your work on this. You should expect to pay the
IA for his time in helping to complete this form.
Without this information - - in a
reasonably complete and legible form - - your aircraft may not
be included in one of the various Amendments to the AMOCs that
are going to be submitted to the FAA, or it may be deferred from
the first round of amendment language that is submitted, while
waiting on a fully completed detailed history of your aircraft.
Submit a copy of this document to the AMOC holder at the earliest possible time.

Questionnaire.) Be sure and keep a copy of the
completed form. Including it as part of your log book records
will be required as a part of the anticipated AMOC amendments.

The questionnaire instructs you to
submit several documents along with the questionnaire. One of
them is a compact disc (CD-ROM) containing all of the FAA

completed DAH questionnaires. Your questionnaire and the CD-ROM
from the FAA should go to the holder of the AMOC for your wing
spar. For nearly everyone in the fleet, that will be one of
