Formation and FAST Information

2004 Revision Summary

In 2004, F.A.S.T. (Formation and Safety Training) made two substantive changes in the reporting requirements for FAST qualified pilots.

First, the language of the reporting form (see Formation Proficiency Report at bottom of this page) is more specific about the proficiency to which the Lead or Check pilot is attesting. The statement on the form now says:

One of the most important responsibilities of the Lead Pilot is to document the currency and competency of the Wing Pilots with whom he/she flies. The Lead Pilot may provide additional training if necessary, and ensures that any pilot he/she has certified as current, has participated in a formation flight in a safe and standardized manner in accordance with FAST standards. By signing this form, the Lead Pilot (or Check Pilot) certifies that he/she has flown with and observed the above and or attached listed pilots, and verified that, on the date indicated on this form, they met those standards. If any pilot in the flight is unable to perform to the required FAST standards, his/her name should not appear on this Report. Total number of pilots being endorsed by this Report, including attached forms is

Signed: Lead or Check Pilot) _______________________

The second substantive change is that Lead and Check Pilots can no longer self-certify their currency. Another Lead or Check Pilot who flew in Lead’s formation as a Wingman is now required to sign off the currency of Lead and Check pilots.

FAST has also asked that the T-34 Association have a specific FPR, with a location and date, for each pilot for whom the Association issues a FAST card. We cannot accept emailed or faxed lists of pilots who flew at an air show (e. g. Oshkosh, Sun 'N Fun) as the basis on which to issue cards. More than four pilots can be certified on one FPR, but the Lead or Check Pilot doing the certification has to have flown in formation with all the pilots being certified.
